This is a professional direct service position which involves program development, oversight, evaluation, and coordination of an assigned service system in cooperation with the appropriate Operations Manager. This position provides leadership regarding a specialized service area while seeking “best practice” models in effort to improve quality and efficiency of services. This position is responsible for the supervision of the day-to-day operations of a group of employees as assigned.
ESSENTIAL POSITION FUNCTIONS (listing Position Specific functions first):
- Document and develop processes and procedures to be reviewed and approved by the Director for implementation.
- Collaborate with the Program Director, to design, develop and evaluate program services, e.g., by completing Needs Assessments and program modifications to meet demands.
- Carry out supervisory responsibilities in accordance with NLCMHA’s policies and applicable laws. Responsibilities include team participation in the process of interviewing, hiring, and training employees. Conduct supervisory sessions, performance-monitoring activities, and professional development planning for team members.
- Complete performance appraisals and competency assessments per agency standards. Effectively address disciplinary issues when necessary. Have the ability to establish measurable performance standards, and to measure actual performance against those standards; the ability to identify necessary competencies and to assess competency; and the knowledge and understanding of basic human resources policies, procedures, laws, regulations, and union contract requirements vital to this role.
- Act as a liaison/consultant to other programs, staff, and community.
- Monitor service delivery and impact of service effectiveness as well as consumer satisfaction.
- Provide training and education and serve as a role model.
- Work collaboratively with the Northern Health Care Management Quality Specialist to improve best practices and participant outcomes.
- Coordinate Person-Centered Planning. In doing so, attend to total spectrum of the individual’s needs, including but not necessarily limited to, housing, family relationships, social activities, education, finance, employment, health, recreation, mobility, protective services and documents as appropriate.
- Participate in Quality Assurance case review procedures, including consumer satisfaction.
- Collaborate with consumers (and staff) in assessing their needs, developing goals, objectives, methodologies, timelines, and evaluation of progress toward goals.
- Request, when necessary, review of an individual’s program plan by the individual’s interdisciplinary team.
LICENSING or CERTIFICATIONS:
- Must have licensure with the State of Michigan as a Licensed Masters Social Worker.
- Must maintain licensure.
MINIMUM & EDUCATION & EXPERIENCE:
- Social Work degree required, five years of long-term care or health care experience preferred.
- Applicants who possess an MSW/licensure and are eligible to provide educational supervision is preferred.
