CMH Services Satisfaction Survey underway through August
Everyone who receives Community Mental Health services funded by the Northern Michigan Regional Entity (NMRE) is invited to share their feedback in a CMHSP Services Survey through August, 2026.
There are three ways to take the survey and share your feedback:
- You can complete the survey using a paper form (click here to download a printable form), which you can return to your provider.
