THIS POSITION CAN BE A FULL TIME POSITION OR A CONTRACTUAL POSITION WHICH WOULD BE NEGOTIATED

This clinical position promotes movement and physical modality to evaluate and treat individuals who have a disease or disorder, impairment, activity limitation, or participation restriction which interferes with their ability to function independently in daily life roles, and to promote health and wellness. This employee reports to the Operations Manager as assigned. This position is not responsible for the direct supervision of staff.

 

ESSENTIAL POSITION FUNCTIONS (listing Position Specific functions first):

  • Conduct physical therapy evaluations to determine functional mobility needs regarding range of motion, muscle tone, muscle strength, gross motor skills, balance, gait issues, musculoskeletal needs, fall risk, etc.
  • Design, revise, maintain, monitor, and evaluate training programs and interventions provided to preserve and improve abilities for independent functioning, such as range of motion, strength, tolerance, fall risk, coordination, and gross motor activities. Maintain records and progress notes for these activities.
  • Administer or supervise the administration of therapeutic treatments or activities.
  • Work in conjunction with the consumer’s family, care givers and health specialists for treatment planning purposes.
  • Provide in-service training for consumer’s family, care givers or others with non-skilled interventions in areas of functional mobility.
  • Participate in interdisciplinary staffing meetings as requested.
  • Ensure physical therapy services meet agency, State, and Federal, and other pertinent standards, guidelines, and policies.
  • Learn/understand equipment procurement process including coordination with vendors.
  • Understand the Habilitation Supports Waiver for persons with intellectual/developmental disabilities.
  • Participate in Quality Assurance case review procedures, including consumer satisfaction.
  • Coordinate Person-Centered Planning. In doing so, attends to total spectrum of the individual’s needs, including but not necessarily limited to, housing, family relationships, social activities, education, finance, employment, health (including special needs), recreation, mobility, protective services and documents as appropriate.
  • Collaborate with consumers (and providers) in assessing their needs, develop treatment goals, objectives, methodologies, timeliness, and evaluate progress towards goals.
  • Request, when necessary, review of the individual program plan by the individual’s interdisciplinary team.
  • Maintain knowledge of mental health symptomology, intervention, and treatment strategies.

 

MINIMUM EDUCATION & EXPERIENCE:

Bachelor’s degree in physical therapy.

One (1) year of post-degree experience in working with consumers of the public mental health system is highly desirable.

 

 

LICENSING or CERTIFICATIONS:

Registration as a Physical Therapist and licensing through the State of Michigan is mandatory.
