Professional position available to facilitate the Wraparound service for children with severe emotional disturbance (SED). Services include child and family assessment and “wraparound” for SED children and adolescents. These child consumers may also be involved with or have needs being addressed by other public agencies, juvenile justice, child welfare, and education. The facilitator will work collaboratively with these systems in Crawford and Roscommon counties, as well as with both the child and adult systems within NLCMHA. Responsible for direct assessment and service provision with families, maintaining program statistics and developing required reports, communication and collaboration with the community and various funding sources. This position will be involved in collaboration and consultation with other community human service agencies and schools. Services may be provided in the office, the family home, or in other community-based locations. Will work closely with statewide Wraparound system through MDHHS.

ESSENTIAL POSITION FUNCTIONS (listing Position Specific functions first):
• Under the direction of the COO for Child and Family Services, will provide individual and family supportive contacts, crisis intervention, coordination of care, and case management services to assigned clients and their families.
• Provide leadership and facilitation to the Wraparound Family Team.
• Participate as needed on the Wraparound Community Team.
• Participate in MDHHS required Wraparound training.
• Complete training and staff development requirements as a Child Mental Health Provider.
• Conduct comprehensive, quality assessments of both children and families.
• Assess each client/family referred, and will develop comprehensive, quality individualized/family community intervention plans.
• Ensure family assessments and reports are completed in a timely manner and reflect delivery of quality intervention.
• Ensure funding reports and other required documentations are completed and submitted according to guidelines in a timely fashion.
• Interface with the business office in monitoring and processing billings for payment of services.
• Attend team meetings, supervision meetings, and other meetings as required.
• Work cooperatively with all community agencies and other providers to promote provision of quality, community based services.
• Participate in quality assurance programs and in periodic evaluation of performance and outcomes of service delivery.
• Comply with mandatory reporting requirements in situations of suspected child abuse or neglect.

QUALIFICATIONS:

MINIMUM EDUCATION & EXPERIENCE:
Bachelor’s Degree in Social Work from an accredited university, or a Degree in Nursing.
One year experience in the provision of direct services to children and families. Must demonstrate a good working knowledge of the DSM-V and multiaxial system. Must have training in Family systems theory.

LICENSING or CERTIFICATIONS:
Must have licensure with the State of Michigan as a Licensed Bachelors Social Worker. Limited Licensed Bachelors Social Worker or licensed RN considered. Must maintain licensure.
