Employees in this clinical position are responsible for ensuring continuity of services, with a primary responsibility for assuring that the five case management functions are carried out: assessment, planning, linking, monitoring and advocacy.  This position functions as an advocate for the participant and a clinical liaison service broker.  This position reports to the Director of Long Term Care & Support Services or as assigned. This position provides no direct supervisory responsibilities. Must strongly believe in recovery and have a commitment to creating and maintaining a culture that expects recovery. Northern Health Care Management program covers 10 counties.

ESSENTIAL POSITION FUNCTIONS (listing Position Specific functions first):

  • Develop, approve, and coordinate the participant’s Person-Centered plan. In doing so, attend to the total spectrum of the individual’s needs, including but not necessarily limited to: housing, family relationship, social activities, education, finance, employment, health (including special needs), safety, recreation, mobility, protective services, and records.
  • Carry out initial participant assessments and planning using a team approach consisting of a RN and a SW as mandated by the MI CHOICE Waiver MHDDS Contract to provide a holistic, person-centered perspective. This also provides a foundation for thorough planning of meeting medical needs as well as environmental and psycho-social concerns. Reassessments will take place per MDHHS contractual standards.
  • Maintain a caseload that consists of medically complex cases, particularly with participants presenting with cognitive behavioral/mental health concerns.  
  • Monitor the operation of the services that are provided the individual in accordance with the individual program plan, requesting services when appropriate, and works to resolve concerns and make necessary improvements as needed.
  • Locate, obtain, and coordinate services outside and inside the MI Choice system as indicated by the individual treatment plan.
  • Establish and maintain an effective liaison between Northern Health Care Management and outside providers.
  • Participates in Quality Assurance case review procedures, including participant satisfaction.
  • Collaborate with consumers (and staff) in assessing their needs, developing treatment goals, objectives, methodologies, timeliness, and evaluating progress toward goals.
  • Provide supportive services to the participant and his/her family. Advocates where necessary.
  • Secure relevant data from the other agencies providing services to keep the individual program plan up-to-date and documents according to standards.
  • Intervene when necessary to assure implementation of the plan.
  • Facilitate the transfer of the individual to another service or agency when such transfer is appropriate to meet the individual’s needs.

LICENSING or CERTIFICATIONS:

  • Must have licensure with the State of Michigan as a fully licensed professional: Licensed Masters Social Worker, or a Limited Licensed Masters Social Worker, Limited Licensed Professional Counselor, or master’s Limited Psychologist considered. 
  • Must maintain licensure.

MINIMUM EDUCATION & EXPERIENCE:

  • Master’s degree in social work (MSW) from an accredited university.
  • Two years’ experience in a mental health related field (preferred).
