Employees in this clinical position are responsible for ensuring continuity of services, with a primary responsibility for assuring that the five case management functions are carried out:  assessment, treatment, planning, linking, monitoring, and advocacy with potentially more challenging consumers.  The Case Manager Specialist is an advocate and service broker for the consumer; serves as a resource and role model regarding case management services and will serve as the primary Substance Use Disorders (SUD) therapist.  The Case Manager Specialist is supervised by the Operations Manager as assigned.  This position has no direct supervisory responsibilities.

 

ESSENTIAL POSITION FUNCTIONS (listing Position Specific functions first):

  • Approve and coordinate the consumer’s Person-Centered plan. In doing so, attends to the total spectrum of the individual’s needs, including but not necessarily limited to, housing, family relationship, social activities, education, finance, employment, health (including special needs), safety, recreation, mobility, protective services and records.
  • Monitor the operation of the services that are provided the individual in accordance with the individual program  plan, requesting services when appropriate.
  • Provide crisis intervention services when appropriate.
  • Function as a primary SUD therapist for co-occurring consumers.
  • Collaborate with consumers (and staff) in assessing their needs, developing treatment goals, objectives, methodologies, timeliness, and evaluating progress toward goals.
  • Provide supportive services to the consumer and his/her family.
  • Secure relevant data from the other agencies providing services to keep the individual program plan up-to-date and documents according to standards.
  • Intervene when necessary to assure implementation of the plan.
  • Facilitate the transfer of the individual to another service or agency when such transfer is appropriate to meet the individual’s needs.
  • Provide services to more clinically challenging cases on the team.
  • Serve as a mentor or role model.

 

MINIMUM EDUCATION & EXPERIENCE:

  • Bachelor’s Degree in Social Work from an accredited university, or a Degree in Nursing.
  • Three (3) years of post-degree experience in developmental disabilities or mental illness areas preferred. Experience as a case manager is desirable.
  • Experience working with co-occurring consumers is preferable.

 

LICENSING or CERTIFICATIONS:

  • Must be a Certified Advanced Alcohol and Drug Counselor (CAADC) or a Certified Alcohol and Drug Counselor (CADC) If the applicant does not have either certification, they must be eligible to be placed on a development plan through MCBAP to be certified as a CAADC or CADC.
  • Must maintain this certification.
