Wraparound Facilitator (Cadillac location)

Requirements of Position – Must have minimum licensure with the State of Michigan as a Licensed Bachelors Social Worker. Limited Licensed Bachelors Social Worker or licensed RN considered. Must maintain licensure. One year experience in the provision of direct services to children and families. Must demonstrate a good working knowledge of the DSM-IV and multiaxial system. Child Mental Health Professional. Experience in the provision of direct services to children and families. Must demonstrate a good working knowledge of community and child serving systems. Must have training in Family Systems theory.

Position Responsibilities – Professional position available to facilitate the Wraparound service for children with severe emotional disturbance (SED). Services include child and family assessment and “wraparound” for SED children and adolescents. These child consumers may also be involved with or have needs being addressed by other public agencies, such as juvenile justice, child welfare, and education. The facilitator will work collaboratively with these systems in Wexford and Missaukee counties, as well as with both the child and adult systems within Northern Lakes CMH. Responsible for direct assessment and service provision with families, maintaining program statistics and developing required reports, communication and collaboration with the community and various funding sources. Services may be provided in the office, the family home, or in other community-based locations. Will work closely with statewide Wraparound system through MDCH. This position is located in the Cadillac office.

Working Conditions – This position may require irregular hours. It may also require travel by personal vehicle if an agency vehicle is not available.
