Residential Services Coordinator MI Choice Waiver Program (10-county location) – part time

Requirements of Position – Bachelor’s Degree in Social Work from an accredited university, or a Degree in Nursing. One (1) year experience in long term care services is preferred. Experience with skilled nursing facilities and licensed residential homes is preferred.

Position Responsibilities – Position is part-time (.60 FTE).  Individuals in this clinical position will work with nursing facility residents, their allies, and facility staff to identify candidates for transition and assist with transition options. Will inform nursing facility staff of the MI Choice options including residential services option. Conduct research and outreach with AFCs and HFAs to inform them of the requirements and processes for becoming a MI Choice Waiver provider and assess their capacity to meet contract requirements for appropriate care for participants. Responsible for ensuring continuity of services, with a primary responsibility for assuring assessment, planning, linking, monitoring and advocacy. Reports to the Program Director as assigned. This position has no direct supervisory responsibilities. This position is located in the Traverse City Office.

Working Conditions – This position may require irregular hours. It may also require travel by personal vehicle if an agency vehicle is not available.
